While various digital platforms exist to support impairment-focused and strategic therapy, many fall short in usability, flexibility, and engagement. Prior to adopting Cognishine, therapists frequently encountered outdated and repetitive tasks that lacked customization, offered limited options for home practice, and provided no tools for tracking client progress.

Client 1: 74 year old male, aphasia and apraxia of speech

Lack of engagement in therapy, despite recognising a need.

Impairment language tasks are hard for this client and evoke errorful patterns. We have successfully therefore used the cognitive tasks on the platform to practice and use his intact thinking skills and gain success as a basis for practicing and improving his language skills needed to discuss real life activities and events. This meant the client had the support of familiar contexts, visual aids on the screen which help his comprehension significantly, and was feeling real success and enjoyment from getting the task right whilst also practicing the strategies which are difficult in saying and communicating what it was about.

Client 2: 69 year old male, haemorrhagic stroke

Moderate-severe non fluent expressive aphasia with apraxia of speech. Experiencing word finding difficulties, semantic and phonological paraphasias with some perseveration on word. Goals identified: to know what to do 'in the moment' when I get stuck; To write a short sentence; To be able to talk more about my everyday life.

We used Cognishine picture prompts (focusing on areas of interest) to ask the client to create simple spoken and written SVO sentences, using this as a step up from more scaffolded therapy tasks such as VNest. We also used the picture prompts to facilitate drawing and gesture practise, building on his skills as a total communicator and working to increase confidence.  We used naming tasks to encourage practise of circumlocution, incorporating the semantic feature analysis model. Social situations images, verb pictures and picture prompts were also used to facilitate response elaboration therapy.

The client enjoyed the activities

Particularly ones with animals and videos which brought to life what he had been figuring out or expressing (dog jumping into a swimming pool for example). Cognishine gave focus, success and an indirect benefit of being able to practice speech and language skills where this was less of the focus. It is worth noting that many of these tasks were handed over to the client to continue practising in his own time via a board that was easily shared with him.

The client gradually became a fantastic total communicator

Over the course of therapy the client gradually became a fantastic total communicator, using his strategies of gesture, mime and drawing brilliantly in conversation. Cognishine was very valuable in providing us with resources to help develop these skills. The client also began to spontaneously generate SVO sentence structures, with increased length of spoken utterance, again a goal achieved that had been supported through use of Cognishine materials.

Cognishine has saved me a huge amount of time in terms of resource preparation. It has encouraged self-management and enabled increased intensity of therapy as clients are able to independently continue with practise outside of sessions. There is a great variation in resources presented via Cognishine. I was able to find resources targeting key interests for all of the clients, for example, from winter sports to favourite musicians/bands.

Conclusion

We certainly recognise Cognishine as another tool in our box for supporting people with aphasia and CCD.  It would be one of the core 'go to' screen based systems we would now routinely think of using with our clients. It is up to date, vibrant, and portable.

Cognishine was a real time saver during the delivery of ICAP. It helped us generate ideas for therapy tasks and gave us access to appealing, contemporary images that clients were very engaged with. It also facilitated easy access to self directed therapy outside of sessions.
